Ordering a local loop
For Lumen® Vyvx®, you can download a form from Media portal to order a new local loop from Lumen. Once you fill in the form and submit it, the Vyvx® Provisioning team processes your request.
To order a local loop:
- Sign in to Media portal. (Get help resetting your password)
- From the main menu, select Orders > Video Local Access Form.
Media portal downloads the local access request form.
- Open the form using a PDF reader (such as Adobe Acrobat).
- Fill in the fields on the form. (Required fields are outlined in red.)
- When you're done, click the Submit to Level 3 button (on the bottom of page 2).
