DIA rates include port, access and logical service for non-burstable access. Rates exclude installation, optional add-ons, building extensions, construction charges, customer premise equipment (handsets and hardware), taxes and fees. Early termination fees will apply for each service component as set forth in Customer Agreement. Customer must remain in good standing; rates may terminate if customer changes account including, but not limited to, service location. Lumen may change, cancel, or substitute offers and services, or vary by service area at its sole discretion without notice. Credit approval and deposit may be required.
